As a Foster Care Case Manager at Health Connect America, you'll play a critical role in supporting children and families within the foster care system—ensuring they receive the coordinated, compassionate care they need to thrive.
As a Foster Care Case Manager, you'll oversee a caseload of children and families, ensuring services are well-coordinated, plans are individualized, and every child's needs are met with consistency and care. This role requires strong organization, adaptability, and the ability to manage multiple priorities in a dynamic environment. You'll be working closely with a variety of stakeholders, often balancing urgent needs with long-term planning. Your ability to communicate clearly, problem-solve proactively, and maintain attention to detail will be key to success.
You'll also play a role in supporting foster parent recruitment, assessment, and ongoing engagement—helping build a network of caregivers who are equipped to provide safe and supportive homes.

In this role, you'll ensure all services and documentation meet Health Connect America (HCA), state, and Council on Accreditation (COA) standards. You'll be expected to maintain high-quality work, meet required timelines, and actively contribute to continuous improvement efforts—ensuring the best possible outcomes for the children and families you serve.
