Job Summary

The Hospice and Palliative Medicine Physician provides the medical component of the Hospice/Palliative Care Programs. The Hospice and Palliative Medicine Physician assists in the maintenance of appropriate medical policies and standards to meet agency, state, federal and accreditation requirements. This position will assist in providing hospice care and palliative care patients in various settings. The Hospice and Palliative Medicine Physician works collaboratively with the Transitions LifeCare medical directors and staff and acts as a liaison to the Physician Community. The schedule for this position is Monday-Friday 8:30am-5:00pm; On-Call rotation; weekends and holidays as requested.

Responsibilities what you do

  • Provide hospice visits and palliative care consultation to patients in private homes, residential facilities, Hospice Inpatient Facility and hospitals as requested.
  • Provide consultative, supervisory and direct patient care utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards.
  • Attend IDG meetings as appropriate
  • Document in patient medical record to meet federal, state, regulatory, and agency guidelines.
  • Participate in activities that educate the medical community, hospital staff and community physicians about hospice, palliative care, pain, and symptom management, communication around end of life decision making and other Transitions LifeCare Programs.
  • Provide education to rotating students, residents and fellows.
  • Participate in physician on-call schedules to ensure that physician services are available 24 hours daily, 7 days per week.
  • Assist in data collection, forms, and outcome evaluation tools for the PalliativeCare Program as needed.
  • Provide support and oversight for advanced practice providers on the hospice and palliative care teams
  • May serve as collaborating physician for advanced practice providers
  • Participate in the development, review, and modification of comprehensive plans of care with other members of the patient care team.
  • Participate in the establishment and review of medical policies and procedures governing hospice/palliative care according to need.
  • In conjunction with the patient’s Attending Physician, certify patient prognosis at the time of hospice admission when needed.
  • Review/recertify patient prognosis for continuing hospice services on HMB/HMC patients as appropriate.
  • Be responsible for pharmacy utilization for patients under your care.
  • Attend weekly IDG meeting as a full and active participant. When the Attending Physician is not available to meet the medical needs of the patient, will provide the staff with appropriate medical direction for the patient.
  • Perform other duties as assigned by Chief Medical Officer

Requirements what you need
  • Meet and maintain credentialing requirements for acceptance to TL and community hospital staffs
  • Graduate of certified medical school with MD or DO degree, followed by residency training and Board certification in a specialty suited for hospice and palliative medicine
  • Licensure in North Carolina
  • Current DEA license
  • Current CPR certification.
  • Maintain CME requirements for hospital staff and NC Medical Board
  • Must work toward acquiring prerequisites for Board Eligibility and passing the National Board Examination in Hospice and Palliative Care.
  • Must have and maintain a valid North Carolina Driver’s License, maintain automobile insurance coverage and have access to reliable transportation.
