LifeBridge Health is a regional healthcare organization in Maryland providing a comprehensive continuum of care, including hospitals, specialty centers, and community health-related operations. We are seeking an ED Case Manager Social Worker to join our Healthcare & Medical Services department and support our emergency department by delivering compassionate, patient-centered care. In this role, you will work directly in the emergency department to assess patients’ psychosocial needs, coordinate discharge planning, and connect patients and families with appropriate community and healthcare resources. You will conduct timely assessments, identify barriers to safe discharge, and collaborate with physicians, nurses, and other interdisciplinary team members to develop and implement comprehensive care plans. You will provide crisis intervention, brief counseling, and support for patients and families experiencing acute medical, behavioral health, social, or substance use crises. The ED Case Manager Social Worker will review clinical information, benefits, and coverage to help ensure appropriate level of care, facilitate transitions between inpatient, outpatient, and community services, and reduce avoidable readmissions. You will maintain accurate and timely documentation in the electronic medical record, track referrals and service utilization, and communicate regularly with community agencies, post-acute facilities, behavioral health providers, and social service organizations to ensure continuity of care. Aligned with LifeBridge Health’s culture of collaboration, innovation, and community engagement, you will actively participate in interdisciplinary rounds, quality and process-improvement initiatives, and departmental meetings. You will help identify gaps in services, suggest innovative approaches to care coordination, and contribute to initiatives aimed at improving patient outcomes, patient experience, and population health in the diverse communities we serve. This position requires strong clinical social work and case management skills, including psychosocial assessment, crisis intervention, motivational interviewing, care coordination, and advocacy. You should be comfortable working in a fast-paced ED environment, managing multiple priorities, and communicating effectively with patients and families from varied cultural and socioeconomic backgrounds.
